The
Atlantic City Medical Center Community Health Services Back On Track
Program
is purposed to identify and provide outreach and referrals for at-risk
or high school dropouts. The program is a high school dropout outreach
and referral service targeting adolescents in the community who
have either dropped out of school or who are at risk of dropping
out. Those adolescents include pregnant and parenting teens, teens
dissatisfied with school including those having academic difficulty
and/or behavioral problems within the school system.
Referrals for this service are obtained from our own Atlantic City
Medical Center's maternity and pediatric units, as well as from
the family planning program. Other contacts with the teens are made
from referrals by local agencies, word of mouth, and from outreach
efforts conducted by the case manager of the program. Teens are
assessed by the case manager and linkage is provided for the teen
